With a strong passion for health equity and experience in clinical settings, policy and data analysis, Cami has cultivated a dynamic skill set focused on practice transformation, program implementation and process improvement. Whether its designing project infrastructure, navigating complex stakeholder landscapes or implementing evidence-based process improvements, Cami is deeply committed to transforming healthcare practices and systems to better serve marginalized communities and advance health justice. 

As a Practice Transformation Consultant at Elevation Health Partners, Cami works on the Value-Based Care team where she supports the implementation of CalAIM initiatives by providing techincal assistance to Enhanced Care Management and Community Supports providers via the PATH TA Marketplace. She drives practice transformation through program development, evidenced based quality improvement models and workflow design and improvement.   

Prior to joining Elevation Health Partners, Cami managed the grant administration of the CalAIM Incentive Payment Program for Enhanced Care Management and Communty Supports providers in San Diego and Imperial counties, on behalf of the regional Managed Care Plans (MCPs). She also served as a Quality Specialist at a Federally Qualified Health Center which exlcusively served individuals experiencing homelessness. 

Oustside of the of the office, you may find her practicing yoga, reading, or spending time with her family and friends. As a Boricua living in San Diego, she often returns back home to Puerto Rico to reconnect with her family and culture.
