With over 15 years of experience in the healthcare industry, David is passionate about leading transformative programs that improve patient health outcomes and provider satisfaction.
As a Senior Practice Transformation Consultant at Elevation Health Partners, David leverages his experience in practice management and provider engagement to support practices in achieving their quality of care goals and initiatives. Prior to joining Elevation Health Partners, David held diverse roles, including managing a portfolio of practices located in 18 states to support risk adjustment and quality of care initiatives, creating a collaborative Patient-Centered Medical Home (PCMH) program within a large safety-net health system, and integrating a comprehensive care coordination program that reduced hospital readmissions and unnecessary ED visits.
David holds two professional certifications: Certified Professional in Healthcare Quality (CPHQ) and Patient-Centered Medical Home (PCMH) Certified Content Expert.
In his free time, David enjoys spending time with his growing family, woodworking, and getting some fresh air on the golf course.
