Jasmin is an advocate for public health who is passionate about promoting health equity and diversity, providing comprehensive and quality healthcare, delivering exceptional customer service, and building healthy communities. She enjoys collaborating and supporting multidisciplinary care teams, integrating innovative evidence-based models into primary care, and using data to improve overall patient care. Jasmin’s expertise centers on social drivers of heath, population health management, quality improvement, and health promotion.
As a Practice Transformation Consultant at Elevation Health Partners, Jasmin provides technical assistance and coaching to Enhanced Care Management and Community Support providers throughout Los Angeles County. Jasmin works directly with clients to build internal capacity to implement the program by strengthening workflows, providing trainings, implementing evidence-based practice improvement techniques, and sharing best-practices.
Prior to joining Elevation Health Partners, Jasmin worked at a large Federally Qualified Health Center in Los Angeles County where she led innovative public health projects including social needs screening and closed-loop referrals, a food prescription program, produce distribution events, and SNAP-ed delivery. She also served as the Quality Improvement Coordinator and supported practice improvement team meetings to improve clinical and operational metrics.
Outside of her role at Elevation Health Partners, Jasmin enjoys traveling, living an active lifestyle, and spending time with her family, friends and her dog Scooby.
