Karen has long been passionate about solving health disparity issues and understands the vital importance of healthcare initiatives in ensuring community success and development. She has served in a variety of program development roles that emphasize collaborative teamwork and creative problem-solving methods to best support underserved populations.
As a Practice Transformation Consultant at Elevation Health Partners, Karen is part of a large-scale practice transformation project that provides coaching to multiple clinic locations throughout Los Angeles County. Karen works directly with clients to improve patient care by implementing prescribed evidence-based care management and practice transformation strategies. Karen is also an assigned coach directly supporting several PCMH recognized FQHCs under the program. A Patient-Centered Medical Home Certified Content Expert, Karen aligns her practice transformation work with PCMH recognition.
Prior to joining Elevation Health Partners, Karen was the Program Manager of Health Education at Northeast Valley Health Corporation. In her role, Karen coordinated the delivery of health education services and programs to health centers and communities. She collaborated with internal and external stakeholders to ensure the success of a broad range of programs and services.
Karen strives to continuously contribute to positive social change through her work. Outside of her role at Elevation Health Partners, Karen enjoys spending time outdoors with her family and her dog Hussle.
