Lauren is a passionate advocate for equitable healthcare and brings over 12 years of experience in the healthcare industry through which she specializes in health information technologies, population health management, quality improvement initiatives, customer success, patient-centered medical home and patient engagement and activation. With her collaborative spirit and commitment to innovation, Lauren is dedicated to supporting community health centers and care teams to advance patient care.
As Principal Consultant at Elevation Health Partners, Lauren leads practice transformation leveraging industry best practices and supporting all stakeholders, including but not limited to executive leadership, administrative and clinical teams, HealthIT vendors, health plans, as well as other consultants, government, advocacy, and community collaborators.
Before joining Elevation Health Partners, Lauren supported the success, implementation and adoption of CareMessage, a patient engagement platform for underserved communities, for over 60 community health centers across the Western U.S. Lauren has also served as Director of Continuous Quality Improvement at Gardner Family Health Network in San Jose, where she led NCQA PCMH L3 accreditation efforts in addition to leading program interventions to improve clinical quality indicators. Lauren is an i2i Population Health Super User, having earned certification for advanced analytics during her time as the Education Program Manager and Consultant.
Outside of the office, Lauren enjoys spending time at botanical gardens throughout Los Angeles with her two young daughters, cheering at her husband’s hockey games and taking walks through the Claremont Colleges. Lauren is also a trauma-informed yoga teacher and volunteer birth and postpartum doula.
